I can't think of a more defining moment for "ER" than the death of Dr. Greene. It's an image that no fan can ever forget, and the use of the iconic song "Somewhere Over the Rainbow," beautifully reworked by Hawaiian artist IZ, doesn't make this pill any easier to swallow.
